Requesting Access to or Copies of Your Protected Health Information (Medical Records):
COVID-19 Advisory: Due to COVID-19, we are only open for medical record walk-ins by appointment at Presbyterian Kaseman Hospital. Patients can make appointments on Mondays from 8 a.m. to 5 p.m. (closed for lunch from 12 p.m. to 1 p.m.). Please call
(505) 841-1944 to schedule an appointment. All other hospital locations are closed for medical record pick-up.
To request that your Protected Health Information (Medical Records) be released to another party, or to obtain a copy yourself, please complete an authorization form.
To request a correction (amendment) to your Protected Health Information (Medical Records), please
complete an authorization form. Your provider cannot consider any changes to your medical record until you complete this form. Submission of this form does not guarantee that your record will be changed as requested.

Request through MyChart
If you have a MyChart account, you can request your Presbyterian medical records for free through the “Medical Records” tab after logging into your account. After you make a request, your records will be available for you to download and print for one month.
You can only request records for yourself or a child under the age of 14 and cannot request records for an adult proxy.
