Pre-Registration Forms

The documents below provide important information, guidelines and resources related to your appointment. They are required to be reviewed and acknowledged for both in-person and telehealth video appointments with your provider. Click on the links below to review these documents before your appointment so you are familiar with them.

If you have an upcoming telehealth video appointment and have received a text message to review these documents, please do so then reply to the text message confirming your review and consent for treatment. This is a requirement and MUST be acknowledged before your provider can treat you. If you are unable to acknowledge these documents before your appointment, your appointment may need to be rescheduled to a future date.


Consent to Treat

This document allows us to see you, treat you and bill your insurance. It also talks about your financial responsibilities should your insurance not cover the full amount.


Abuse and Neglect

This document provides you with information should you need to contact a state agency regarding abuse, neglect or misuse of personal property. There are numbers for Child Protective Services, Adult Protective Services and the New Mexico Department of Health Improvement.


HIPAA

This is our privacy notice (Health Insurance Portability and Accountability Act). This booklet explains how we may and may not utilize your protected health information.

These documents explain how Presbyterian bills Medicare and TRICARE.


Patient Rights and Responsibilities

This document explains both your rights and your responsibilities as a patient at Presbyterian.

Independent Contractor

During the course of medical treatment with Presbyterian, care or treatment may be provided by independent contractors; that is, doctors and/or other providers who are not employees or agents of Presbyterian.


Financial Aid

As part of our charitable mission, Presbyterian offers help and advice to our patients who are unable to afford their medical bills.
